Bariatric and Specialty Dolphin FIS Configurations

Width is not capacity

These are two separate specifications and people needing one often ask for the other. Width is about room — room for a broad set of shoulders, a large abdomen that shifts when someone rolls, contractures that hold a limb out from the body, or two caregivers working on the mattress at the same time. Capacity is about load. A person can be well within 500 lb and still have nowhere to turn on a 35-inch surface, and a person over 500 lb needs the rating regardless of how they are shaped.

So the 42-inch surface answers a room problem at the standard rating, and the 48-inch answers a load problem. Joerns states the system is designed as a therapeutic mattress system for patients weighing up to 1,000 pounds and is customizable to fit bed frames, including bariatric bed frames, up to 48 inches wide. Above 500 lb, the 48-inch is the only configuration in this line, and it comes with a frame requirement rather than a preference.

Whatever the surface, the capacity figure is the mattress figure. Joerns footnotes every one of them: total weight must not exceed the bed frame manufacturer’s specified load capacity. Bariatric frames publish a patient limit and a total limit including mattress and accessories; the patient limit is the one to hold the person’s charted weight against, and it is frequently lower than the surface’s rating.

Bariatric orders are frame orders

The surface is the light part. Joerns lists the therapy mattress at 22 lb and the AutoVector control unit at 10 lb, which is why people are surprised when a five-figure therapy system arrives in two manageable pieces. What is not light is the bariatric frame underneath, and that is where a bariatric order is won or lost. A 48-inch surface needs a 48-inch deck rated for the person; a 42-inch surface needs a 42-inch deck. Neither is adjustable by arranging it carefully.

Fit on a wide bed matters for the same reason it matters on a narrow one, and the warning is the manufacturer’s rather than ours. Joerns states that the risk of entrapment can arise when equipment is placed on bed frames that leave gaps of even a few inches between the therapy mattress and the head panel, foot panel and bed or side rails, and that the equipment is not to be used when such gaps are present. Three inches of gap at the hip on each side of a 42-inch surface sitting on a 48-inch deck is exactly that situation.

Then there is the room. A wide bed needs floor space on both long sides if two caregivers and a lift will work there, a route in that frame sections can turn through, and a floor that will take the loaded bed where you intend to put it. We ask about door widths, stair turns and landing corners on these orders because the questions are cheap before delivery and expensive afterwards.

The hours out of bed: seated and transport surfaces

A therapy surface in the bed protects the lying hours. A person who then sits for four hours a day on an ordinary foam cushion has four unprotected hours, and seated loading concentrates weight over a much smaller area than lying does. That is the gap the Dolphin wheelchair cushion and stretcher pad exist to close, and they are two genuinely different products with two very different ratings.

The wheelchair cushion is 17 by 17 by 4 inches, weighs 5 lb, and Joerns gives its maximum safe working load as 250 lb — the lowest figure in the range by a wide margin, and a firm limit rather than a guideline. The stretcher pad is 36 by 76 by 5 inches, weighs 14 lb, and Joerns gives its maximum as 700 lb. Joerns notes on this line that the 12 hour battery life allows for active therapy even during patient transport, which is the clearest statement of what a transport surface is for.

What a wheelchair user should sit on is an occupational therapy or seating clinic decision, made against the chair, the posture, the transfers and the skin history. A 17-inch cushion is one seat size among many, and a therapy cushion in a badly fitting chair does not fix the chair. If a seating clinic is involved, their specification governs and we supply to it.

Pediatric and crib systems

Joerns states that the pediatric system accommodates patients from 5 lb to 300 lb, with 5 lb given as the system minimum. That range covers neonates through to adolescents and young adults still in pediatric services, which is a wider clinical span than any other product in the line. The crib therapy mattress comes in two published sizes: US at 29.5 by 57 by 5 inches, International at 28 by 50 by 4 inches. Seven inches apart in length, and not interchangeable.

Crib fit deserves its own sentence. Crib internal dimensions vary by manufacturer and by market, and a crib surface that does not fill the crib is a safety problem before it is a therapy problem. Measure the inside of the crib base, width and length, and send us both figures before ordering. Joerns’ entrapment warning applies with more force in a crib than anywhere else on this site.

We are deliberately cautious in pediatric copy. A great deal of pediatric pressure damage is device-related — tracheostomy flanges, splints, casts, gastrostomy buttons — rather than caused by the surface underneath, in which case a therapy mattress does not address the cause. What a child needs is the pediatric team’s decision: the wound or ostomy nurse, the physician, the complex care coordinator. Bring us their specification and we will match it.

How these are paid for, and what we will not claim

Everything in this group is bought or rented privately. We do not bill Medicare and we do not submit claims on your behalf. Fluid immersion simulation has no HCPCS code of its own, and a Dolphin FIS is not an air-fluidized bed: E0194 is defined by warm filtered air forced through silicone-coated ceramic beads, and CMS includes heavy-duty and bariatric air-fluidized devices under that same single code — which is precisely why people looking at a bariatric therapy surface sometimes assume it applies here. Joerns’ own claim is that the system delivers many of the best elements of air-fluidized therapy, such as three-dimensional volumetric engagement and the elimination of gradient shear forces. That is a comparison to a technology, not a coding equivalence.

Where a covered route exists, it is usually one tier down. Group 2 powered surfaces — E0277, E0371, E0372 and E0373 — are covered where a beneficiary meets at least one of three conditions: multiple stage 2 ulcers on the trunk or pelvis that have failed to improve over the past month on a comprehensive ulcer treatment program; large or multiple stage 3 or stage 4 ulcers on the trunk or pelvis; or a myocutaneous flap or skin graft for a pressure ulcer on the trunk or pelvis within the past 60 days with a Group 2 or Group 3 surface in place immediately before a discharge in the past 30 days. A Standard Written Order must reach the supplier before a claim is submitted. If your clinician can document any of that, ask them about a Group 2 surface before you talk to us.

On the private side, a mattress is a dual-purpose item, so it is not on a pre-approved HSA or FSA eligibility list on its own. A Letter of Medical Necessity from a licensed clinician — naming the condition, the item, why it is needed for that condition and a duration — is the mechanism that changes that, and your plan administrator applies the IRS rules and decides. We provide an itemised receipt suited to substantiation. We do not draft the letter, pre-fill one, or tell you a claim will be paid. Veterans’ health networks purchase and lease through their own contracting channels rather than Medicare DME rules; we sell and lease into that route and will explain the process we actually run rather than naming a schedule we cannot show you.
